The Patient’s Medical Home is a model based on your health care needs. The first pillar of the Patient’s Medical Home is patient-centeredness, which means your family physician and other members of your health care team are responsive to your needs, preferences, and experiences. The team at your PMH will work to help you better understand areas of your health that need further attention and will seek your input on your care, to improve your health.

PMH toolkits are designed to describe specific, actionable steps physicians and their teams can take to align their practices with the PMH principles. Our first kit focuses on changes that can take place without the necessity for coordination with upstream health organizations or funders, or more comprehensive system supports. There is an individual kit for Manitoba, reflecting the variety of resources and policies across the country. They can be accessed on this page, and also under the provincial pages in the PMH in Canada section of the CFPC website Implementation Kit Manitoba College of Family Physicians (patientsmedicalhome.ca). Future kits will be developed in response to identified needs.
